• Simply download a claim form from HIF’s website and email a scanned signed copy of your claim form and associated receipts
• Fax a signed copy of your claim form and receipts to (08) 9328 1685
• Post your claim form and receipts to: HIF Claims Department, Reply Paid 84423, Perth WA 6847

For hospital claims, the hospital usually sends the bill to HIF for payment. You may still incur out of pocket expenses, depending on the arrangement your insurer has with your specialist and hospital.
